The data is recorded manually and automatically by a Computer Aided Reporting Application (CARA), and by interviewing patients about their activities. The demographic and socio-economic data of the patients is collected using a questionnaire that consists of eight questions. The questionnaire includes socio-economic questions that are general and mostly related to education. Demographic data is collected using the General Household Questionnaire (GQH) (Statistics Netherlands, 2019).
